Population-Based Care Coordination  Program Description

The area of concentration in Population-Based Care Coordination will focus on community level interventions that promote health and disease prevention activities that influence the community’s overall health profile. The new concentration will prepare nurses for roles that will be in greater demand to address the requirements of the Affordable Care Act. Nursing professionals will gain the appropriate skills that will enable them to make ongoing changes in the health systems they serve, as well as acquire the knowledge and abilities to adapt to future challenges in nursing.
